You don't need any documentary evidence to change your title, this can be requested when asking government agencies or companies to change your name, which you will need a Deed Poll for.
For example, you will have your Deed Poll stating that you'd like to change your name from X to Y. When sending this to government agencies or companies you'll also confirm the name change and title change with them individually.
